Required Items for Students Entering the UPK Program:

• Birth Certificate
• Updated Immunization Record
• Physical Form (CLICK HERE TO ACCESS THE FORM) – Completed by Doctor

In addition to the above requirements: please show 3 (three) Proofs of Yonkers Residence which have your address on it. The documents must be dated within the last 30 days.
